Q on 20 amp circuit and branches

Hi,

Yesterday, I repaired an open neutral problem in my home, and noticed some things I wanted to ask about.

1. For a 20 amp circuit, the feed wire needs to be #12, correct?

2. The first receptacle needs to be 20 amp, but subsequent branched receptacles can be 15 amp, correct?

3. What if the first device wired to on the 20 amp circuit is a lighting fixture, and from there it branches to the 20 amp receptacle?

4. Is there a code-based, or "good practices" based limit to the number of branches that can be fed from that first 20 amp receptacle? The open neutral was caused by having 5-6 wires wire-nutted together with insufficient twist. Over time, one fell out.
