Attached sketch shows utility xfmr and 120/208V three phase 2000A switchboard all outside. I have one 1200A feeder breaker from switchboard going underground outside coming up side of building exterior walls to MLO 1200A panelboard next to exterior walls of building labeled cluster #1

The 1200A MLO panelboard has four feeder breakers 400A. Each feeder breaker go underground and come up along side of building first floor into meter stacks.

Question:

1. Each 400A underground feeder when they enter meter stack room from outside first floor need disconnect or not?

2. Is each 400A underground feeder outside feeder need to comply 225 even though the 1200A panelboard is next to wall and has grounding electrode system?

Your drawing is not clear. Try using blue line graph paper, or better yet Clear Print, as those lines don't copy.
