Question off of masters exam

I recently took my masters exam in Wisconsin. I had a question on the test that I never could find the answer to and was seeing if anyone knew where to look. It asked, On a 100a top fed panel, what is the minimum spacing between phase lug and the top of the panel? I looked at table 408.5 and none of those distances matched what was on the test. 2008 code cycle
 
Probably looking for the dimensions from Table 312.6(A), but not really a question that should be asked for a panel that was built by a manufacturer.
