Existing residential installation circa 1960s
Fused Service disconnect (150 amp) adjacent to meter with 3 conductor cable feeding across the house to what used to be the service and is now a sub-panel w/ no main equipment grounding conductor and with circuit neutrals and equip grounds on same neutral bar.
Would it be compliant to simply run a #6 EGC along side the existing 3 conductor feeder from the disconnect to the sub-panel?

Kind of not, but I would rather see that then no EGC in most instances. Other question is what kind of three wire cable?
 
It was an old cloth covered type cable with 3 conductors of #1 or 1/0 cu RBW. I haven't seen that type cable before. It may have been an early type of SER similar to the old 2 wire with braid service cable in "60s houses or maybe just a large size of the early non-metallic cable w/o ground.

Was the neutral insulated was one of my main concerns for asking?
 
Yes....3 insulated conductors. Isn't there something in the NEC regarding all conductors of a circuit or feeder being in the same raceway or cable assembly? And would that apply to the EGC as in this situation?

Yes, 110.54(B) [2011], but with specific exceptions for branch circuits which did not have an EGC originally, 250.130(C). A new EGC for an old circuit is allowed several specific alternates to running a wire or raceway back to the panel. Some of them, by their nature, could not run in same raceway or cable without rewiring, so there is presumption that a separated wire is allowed.
