Receptacle Cover Plate installation requirements

When the cover plate is installed on a residential receptacle, is there a standard of workmanship or NEC code requirment that addresses the way in which the receptacle must extend through the cover plate?

Cover must be flush to wall and if to far back a spark guard is required

It's not in the code (other than the general connection to our having to follow the manufacturer's instructions). Standards of workmanship are very subjective; I don't know of any organization that publishes such standards. For my money, I would like to see the receptacle pulled through the cover plate at least far enough that there are no gaps through which any foreign materials can get into the outlet box.

The code reference is 406.4 (D). There is a standard depth that an attachment plug must have available for a proper connection, and the placement of the receptacle face controls that depth. The receptacle face must be flush or project from faceplate...

I was gonna stick up for you Charlie but it's right there in the index. :p

Why hasn't anybody mention 406.5?
