Does a recessed can above the 8' minimum of 410.4(D) (2002 Code) require GFCI protection?
Does a recessed can below the 8' minimum require GFCI protection?

Ok, does any fixture above a tub or shower ever require GFCI protection, and if so, when?

Only if it's required as part of the listing by the manufacturer. Usually it isn't. It's not a specific NEC requirement.

Here we are required to put the exhaust fan on gfci protection. If its located above the shower. thats the only one though. Shower trim on a can

Most exhaust fans in a shower or tub area are required by the manufacturer to be GFCI protected. It isn't a specific NEC requirement.

You could put them on gfci but doing so is as much a hazard as not.If it trips they could get hurt bad getting out of the shower.I think no and then your protected by nec.
