I am looking for clarification about 3 wire and 4 wire remote distribution panel wiring. Is 4 wire the requirement in all setups now? Hopefully I asked this question in a way that makes sense.

i'll take a stab here and limit my answer to buildings or structures supplied by a feeders (250.32) and note that as of the '08 Code a equipment grounding conductor must be installed.
(You may no longer used the grounded conductor as an equipment ground path).

If by 4 wire you mean L-L-N and ground, the answer is generally yes for new installs. However there are cases where 3 wires (L-L- and N) was acceptable under previous code cycles for seperate structures.

You were required to do it this way for a LONG time if it is not a seperate stucture. There could be some old buildings that could have been compliant but if they have the original wiring, the wiring is probably so old it needs replacing anyway.

In any case, the ground does not have to be a wire. It could be metal conduit or EMT.
