Are you asking if all the existing wiring needs to be upgraded?

That's too broad of a question to answer. It all depends on the structure, how it's wired, and your local codes. Chances are, you only need to deal with the area you're working in.

I'd suggest you call your inspector.

Interesting question. You should refer to any building code that is in effect in the jurisdiction where the work is contemplated. The building code will determine the nature of the work. For example, it might be called a repair, in which the replacement of receptacle outlets would be required to comply with the electrical code, including replacing non-grounding type with gfci-type and dealing differently with the grounding of frames of ranges, etc. Or the work might be called alterations, additions, relocated buildings, changes of occupancy or even historical building considerations. Look to the building code first. :)
 
Around these-here points (VA), if you remodel 50% or more in floor area, the entire structure must be brought to current code standards.
