In an old fuse based panel, there is often a fiber washer which separates the center conductor from the screw base.
I'm looking at a panel right now where that fiber washer failed due to arcing of a loose fuse. Now electricity can bypass the fuse(!).
I'm not going to attempt to fix that particular unit. But what about the others? Are replacement insulating washers still sold for older panels?

In this case the panel is heavy duty commercial unit from PARKER ELECTRICAL MFG. CO, Oakland California (PEMCO). It is in fine condition for it age, except for the issue described. Note the discoloration in the upper fuse bay:
The unit is a 1925 original. The building is fully sprinkler protected. The panel is in use. There's wiring of every era in the building.
You don't have to post that a new panel is needed.
