Requirements of a Service Entrance

I know the N-G bond occurs at the service entrance location. What are the requirements (OCPD, current limiting device, etc) of a "service entrance"?

N/G connection is made at the disconnecting means if the over current protection is not part of that disconnect it needs to be adjacent to it..... 230.90 I think

single family dwelling, manufactured home, commercial, industrial?
