I was looking to add a Hot tub to an existing home. if I use the alternate load calculation where take the 1st 10k at 100% and the remainder load at 40%.

Do you separate the hot tub load and calculate at a 100%?
Like this:
30.000 watts total
10,000
20,000 * 40% = 8,000
18,000
or

Do you add it to the total load where the 1st 10k is 100% and the remainder is 40%?

19000 watts total 10,000
40% 3,600
hot tub 11,000 100% 11,000
24,600

help is greatly appreciated.

The latter. Add it in and then calculate.
