Rules for receptacles above ceilings

Can someone give me a quick reminder about receptacles above ceilings? (I know this is a common topic but I have a particular question)

1. Are plug in power supplies with low voltage cable OK above accessible ceilings?

While we are on the topic:

2. Are receptacles OK above accessible ceilings.

3. Is 120V plug and cord to something, OK above accessible ceiling?
 
1) Yes.
2) Yes.
3) No, flexible cords are not permitted.
Kind of makes condition in question 1 the only practical reason for a receptacle above the ceiling doesn't it? Or at least for an item that basically has the plug molded into the body of the item being powered and not something supplied via a flexible cord

Only other situation I can see is a service receptacle for some sort of tooling like the service receptacle for a outdoor unit. I don't know of any though.
