safety ground vs. system ground

Should a safety ground be isolated from the ground that is fed along with the phase lines? I ask because I am curious about safety concerns with miswires such as ground and neutral lines being bonded together beyond the main service. If there was a fault, would the safety ground become a concern?

If by the term "safety ground" you mean the equipment grounding conductor (EGC), and by the term "ground that is fed along with the phase lines" you mean the grounded (neutral) conductor, the answer is - yes and no.

This is a difficult subject to adequately cover in a forum like this. I have started to prepare a basic tutorial on the subject.
You are welcome to have a look at it, and maybe then come back with some specific questions.
