is there a geneal industry OSHA eletrical standard or code requiring a posting(signage) of required PPE for voltage next to electrical panels or breakers?

As of today the only required information is that an arc flash could occur. This is in 110.16 of the NEC.
There is no requirement in the NEC, NFPA70E, or OSHA for exact labeling requirements.
However, the next edition of NFPA 70E may include some requirements.

Yes, the 2008 70E will require the labels to list at least the arc flash boundary and the Ei or the minimum PPE requirements.

Does 70E require voltage rated gloves and PPE for switching circuit breakers in a panel with the cover on?

V rated gloves - no
PPE - depends on the breaker, refer to table 130.7(C)(9)(a) and 130.7(C)(10) in the 2004 NFPA 70E. You need at least long sleeve cotton shirt and pants and safety glasses.

RAB only applies to exposed live parts and the OP stated the breaker operation was with the covers on.

Remember when quoting NFPA 70e that OSHA in their final rule published in Feb of this year, formally referenced 70e, but referenced the 2000 edition, not the 2008 edition, so although it is a much more thorough document, it is unenforceable. Our company wrote our spec around the 2000 version.

The final rule that became effective 8/13 only covers the installation section of the 70E, phase 2 will cover the SWP's, PPE, etc of the 70E , who knows when that will be released and what version it will reference. The 2004 70E is still enforceable by OSHA through the same general duty clauses as before.
