Transformer and ped with meter and 200 amp ocb belongs to the Utilities. Once again the (A) is that location. The wires (conductors) leaving going to the building or structure is (B) to arrive a the (C) service equipment where we would start (D) if we installed a sub-panel (4- wire) This is as simple as a,b,c,d from Article 100 2008 NEC.
