I know NEC 230.91 states that the service overcurrent device shall be located immediately adjacent to the service disconnect. Would it be code compliant if there is a service entrance rated, non-fused manual transfer switch ahead of the main circuit breaker/panel serving the building? The MTS is on the outside of the building and the panel with the main c/b is located on the opposite side of the wall, inside the building.

IMO, Immediately adjacent thereto means right beside it, not on the other side of the wall.
Though there are some who would argue the definition of adjacent.
 
I don't think you will find a service entrance rated switch that doesn't have fuses or a circuit breaker as part of the switch.

I thought a service rated switch had to have integral OCP. Maybe that only applies to automatic switches, or maybe I'm just mistaken.

No, Most of the ones I look at only require that you use the neutral bar that is listed for the switch.
But it says on the label: suitable for use as service equipment when......
 
It sounds like you either need a transfer switch with an integral OCPD (and service rated) to right a previous wrong. Who is the AHJ?

I have install several of the same type we are talking about. I have been give the interpretation by our state inspectors as long as that switch is outside the building it is OK but the service conductors must land at the closest point of entry in equipment having OCP.
