Does the NEC require a non-utility transformer (separately dervice system) inside a building to have a service rated disconnect / overcurrent device. For example would a 100A main circuit breaker panelboard on the secondary of a 30kVA transformer be required to be marked as "Suitable for use as Service Entrance" or have some other service rating?

Short answer is "not required".
(Under some strange scenario, if your secondary panel was in a separate structure, Service Rated would be required)
