does one need to be isolated neutral recept.?

Denis, are you asking if you can have 2 circuits in the same 4x4 box? or two circuits - one from each of two different services in the same 4x4?
Your terminology is making this hard to understand.
 
short answer..
other than on a MWBC, I woud think you would always isolate your neutrals on receptacles.
as far as the 4x4, you can mix circuits (other than some emergency circuits)

assuming I understood the question....
