service conductors sizing

I have two inspectors saying two different things. One said that you size the secondary conductors to the calculated load and the other said you only do that in the event that there is more than one main disconnect and if there is only one then you size it to the breaker. Acording to 230.42 I agree with the first one. Who is right?

Single overcurrent device requires that the service condutors be rated for that single device.

With two to six over-current devices you are allowed to reduce to conductors down to match the calculated load.

See exception 3 of 230.90(A)

Exception No. 3: Two to six circuit breakers or sets of fuses
shall be permitted as the overcurrent device to provide the
overload protection. The sum of the ratings of the circuit
breakers or fuses shall be permitted to exceed the ampacity
of the service conductors, provided the calculated load
does not exceed the ampacity of the service conductors.
