Service disconnect on exterior

I have a service disconnect on outside of building but my panel is 100 feet away inside the building. Do I need another disconnect in sight of my subpanel?

No main required. The feeder to the panel has protection where it originates at the service disconnect.

Either, within the same structure. Main breaker panels sometimes cost less, and offer convenience.

Agreed, MLO is fine (provided the panel Buss rating is not less than the overcurrent protection provided by the outside disconnect)
