I am confused about SEC's. If I have a weatherhead going to a CT cabinet then to a disconnect and from there to a main distribution panel (MDP) when exactly do the SEC's end or rather, which of those sets of wires are SEC's?
 
From Article 100.

Service-Entrance Conductors, Overhead System. The
service conductors between the terminals of the service
equipment and a point usually outside the building, clear of
building walls, where joined by tap or splice to the service
drop or overhead service conductors.

Service Conductors. The conductors from the service point
to the service disconnecting means.

Is the MDP considered part of the service equipment?
 
In your case, most likely not.
It would appear that your disconnect is the "first means of disconnect" and your "service" stops there.

Let me ask you this, If I had a parallel wires feeding the MDP from the disconnect (four pipes) would I need a ground wire in each pipe or could I put it in just one of the pipes?

You would be required to have a equipment ground in each raceway.
