Service-entrance elbow

Hi
Section 314.5 makes note of a "service-entrance elbow". Does anyone know the history of this term? Such as:
Was it ever a requirement to use a service-entrance elbow on service conductors?
Is this term referenced anywhere else in the code?
How did this term come about?

A service entrance elbow is an LB fitting, with a built in meter offset, so that the pipe will be tight against the building. I bust off lots of these doing service changes on 1945-1960 vintage services, but I have never seen a new one for sale.
