Service Entrance Lightning/Surge protection

Is a lightning arrestor or surge suppressor required to be installed on all services per code? If so can I get a reference.

No. Not per the NEC. Check your local requirements and service standards from your local power company.

Surge Arrestors are capable of being installed ahead of the disconnect. Transient Voltage Surge Suppressors (TVSS) must be stored after the disconnect.
