Do the same service entrance requirements apply for MV as they do for LV systems? In particular do the same requirements for service entrance conductors apply for MV, and do the same requirements for service entrance switchgear in that the MV switchgear must be rated/listed for service entrance and must have a main bonding jumper installed between neutral and ground bus?

It is similar but different. Service is probabaly from a multigrounded neutral system and you will use a single-point grounded neutral system supplied from the multigrounded neutral utility to distribute power. Review 250.184(B) and note the terminology used.
