A customer wants to raise his overhead service because he wants to build a covered patio underneath his current service. If I understand the NEC correctly we will need 3' clearance above the covered patio. Is that correct?
His mast currently passes through the eve of his house then hits the meter. To avoid any roof repair I would like to find a "service mast approved fitting" (coupler) and just add a coupling and extend the mast about 3'. Also adding a guy wire support. Does any one see any problems with this.

Check with the Poco. I understand that most of them do not like a coupling at the top of the mast.
Not sure what the clearances are.

I believe it shall be 8 ft. Per 230.24 (A) and it shall be maintained for a distance of not less than 3 ft. in all directions from the edge of the roof. Old installation was correct because you didn't have patio, and 18 in. thru overhang is OK. If the roof have slope of 4:12 or greater you can have 3 ft. above roof, but generally patios don't have so much of slope. It is about 1;12 or 2:12 max.

You need to look at 230.28(B).
230.28(B) Attachment. Service-drop or overhead service conductors shall not be attached to a service mast between a weatherhead or the end of the conduit and a coupling, where the coupling is located above the last point of securement to the building or other structure or is located above the building or other structure.
