New 200 amp service. can i install a 200amp meter disconect combo unit on outside then a 200 amp main lug[no main] in the house?

Are we assuming outside means on the structure or do you mean detached from the house?

It's allowed if you treat the conductors between the main and the MLO as a feeder which means separated grounded and grounding conductors.
