Will it be code violation if sewage and water lines run above or below the underground electrical duct bank (with 480V wires), the electrical ductbank is the service feeder to the building. Is there a minimum separation distance requirement between elec. ductbank and sewage and water line conduits?

No minimum separation is required by the NEC.

If they are the service conductors, sometimes the utility will have rules regarding clearance around their ductbank. Not in the NEC.

That's a design issue but it is something that may want to be considered for the reason you've mentioned.

Typically water is above sewer or has horizontal separation, dictated by those utility requirements. NEC does not address but if service conductors POCO may have requirements. Duct bank would not require maintenance but water and sewer can have taps made at later date.
