Siemens Sentron Bus Duct Failure

The question I have is. I did a install of a sentron system. We tied it into existing XLX system. The whole job was designed by Siemens . In one year time we had 2 joint failuers. The 1st time it happenend Siemens told me we missed torquing a joint. Did not believe it but they are the engineers. So we installed the new replacement section under the supervision of Siemens and the torqued what we did so we would have no issues. 3 days later it failed again. has anyone ran into this problem with this product. Any help is greatly appreciated
 
Not with Siemens, but our company ran into a GE bus duct with a manufacturing defect where the supplied flange bolts had shanks that stopped the nut from reaching proper compression torque. So the nut would stop, the wrench would click, but you still had a loose connection. GE found this after a similar repeat failure at a customers location.

In your case it sounds like the gear should be assembled, the section should have a conductor-resistance test performed, and/or this thing put under load and an infrared survey done.
