Simultaneously disconnect

Does NEC2008, Section 680.12 Maintenance Disconnectiong Means require all utilization equipment (other than lighting), to be simultaneously disconnected from its ungrounded conductors, all equipment all at the same time? Or does it require just each piece of utilization equipment, on its own, to be simultaneously disconnected from all of its own ungrounded conductors?

The way I see it is that you have a choice to provide "one or more" means to disconnect the ungrounded conductors. At the end of the day every piece of utilization equipment must have a means to accomplish this.

The words say "one or more (disconnecting) means." That tells me that if you have ten items of equipment, you can install ten disconnecting means. Each of those has to turn off all of the power to the one item it serves. You can, if you prefer, have one disconnect that turns everything off, but that is not required.
