Situations where mechanical equipment is not bonded

Have you seen situations where mechanical equipment is installed ungrounded? not bonded? insulated?

I can think only of the dielectric union at both H and C on a water heater.

Is there a Code section that discusses/ allows equipment to not be bonded, other than 250.6(E)?
