Sizing 3-Phase 12.5KV Conductors

I am installing 3 phase 12.5KV cable in 1-5" conduit and looking for the correct Table in 310-60 to get my ampacity. The details don't show 3 phase circuit in 1 conduit. I am using copper so I assume I size them off of T310.77

If by 3 phase 12.5kV cable you mean 3-conductor cable, in "isolated" conduit, use Table 310.75 [2008] or Table 310.60(C)(75) [2011].
