Sizing MV wire

Recently got burned (figuratively) with the whole 90 degree wire/75 degree terminal issue...

How does one size medium voltage wire? I would imagine the rules are slightly different for MV vs LV

In NEC code book, look at Table 310.104(C) and the tables that follow it.
In general, the MV cables tend to carry a lot more current than their LV counterparts per their rated size.
