I need a little help am trying to overcurrent protection for 4/0 primary cable voltage is 13.2 k the insulation is 133% the cable is feeding a existing gear. The cable is in 4" conduit .The fuse protection is currently at 250 amps can I use The 90 degree column in article 310-15 (b) 16? because primary cable has a higher insulation rating .

310.15 (B)(16) doesn't apply to conductors above 2000V. You want to look at 310.60.
