Sizing the service entrance conductors

According to table 310-16 and table 310-15(b)(6). If we had 75 C (165 F)THWN copper(1999 nec)what table do we size the service entrance conductors for a 100amp service and 200amp service. Then go to table 250-66 and 250-122. Example 200amp 3/0 (table 310-16) and 310-15(b)(6)copper 200amp 2/0?

Table 310.15(B)(6) is for 120/240-Volt, 3-wire, Single-Phase Dwelling Services or Feeders only. If your installation qualifies, then this table can be used.

All other services, feeders, and branch-circuits must be sized per Table 310.16. :)
