Sleeves through Concrete wall underground

Does anybody have experience installing conduit sleeves in a concrete form underground. For example 6" rigid sleeve for 4" PVC through wall. I need to make sure they are level and straight and I intend to "sandwich" them between the forms using 3/8" threaded rod, 4s blank with KO on each side of the form and square washers and nuts to squeeze it all together. I haven't performed this task before and I'm looking to see if anybody has more clever procedure. The conduits will be suspended above ground.

If this is underground you may need a sleeve with a water stop if this is going to be link sealed.
Otherwise it may leak around the outside of the sleeve.

Thanks, we are planning on using link seals. That is one of the reasons it's so important for me to get these in right.
