Solar disconnect line load

I seen solar at a house today and had a question about the disconnect let me give you some details below

EMT coming off of roof from panels
Connects into combiner
AC output from combiner goes to line side of fused disconnect (lugs shielded plastic)
Wire coming from Utility power panel (hot tapped on line side before main) is on load side of outdoor solar disconnect. Lugs not shielded on load side so always hot even if you shut off disconnect or main panel.

Is this done commonly?
This was a major Solar company?
Or is there a special disconnect that should be used for this purpose?

The disconnect was installed backwards, the utility side should be the line side, as you concluded. That's the only way to make it safe to change fuses when the disconnect is open. The grid-tie inverter will automatically deenergize its output when it loses the grid reference.
