I would SO want to do an autopsy on that!! What is the mechanism that disconnects inside the device when faulted or tested? Maybe that is partially activating because of a broken or loosened mechanical part inside the device.

I believe the guys from my old company will do the job but it may not be for a month or so.... I will try and remember to get the gfci from them

Thanks!! I love looking a the innards of failed devices. It really helps my troubleshooting skills to see 'crashed' devices disassembled.
