spacing on a pole line

Where would I find requirements for spacing between 5KV, a second 5kV 3 phase line and then 120/240 and below that telephone.

The 5kV are both Hendrix so I suppose they would be helpful where that is concerned. But what code defines this stuff.

I agree with rick5280. See NESC-IEEE C2 the last edition[2017].
In my opinion this rule would be relevant[on my 2007 edition:ashamed1:]:
Rule 235???. Clearance for wires, conductors, or cables carried on the same supporting structure.

Also note this may get into PSC regulation of pole use. Google up "one-touch, make-ready" for some insight into the turf battle for pole space in places Google Fiber is deploying.
