It has been question that a lightening strike caused a generator and main switchgear to malfunction and cause damage to the control circuit, but there has not been any proof to that really happening. On a service with 2000amp main with backup gen should there be some type of tvss or spd ahead of main breaker to protect equipment ? Also could this damage be caused from the inside equipment ? One other question if it was a lightening strike should"nt other things such as telephone scada be affected ? Thanks again

TVSS protection is not a requirement it is a design choice.

Telephone equipment does not have to be damaged - depends if the surge makes its way to the equipment somehow. It is common for it to be a casualty in lightning events however.
