NOT INCLUDED is 200 A meter center & a 125 A load center, AC System. Also my question if you can answer is? Do contractors normarly charge appliance circuirts in the kitchen or is the SQ FT price is included in the proposal or do they charge for each appliance circuit for example: disposal, dishwasher, micro/hood, range, dryer, refrigerator

Neither way, EC's that have been in business for a while know there is no such thing as square foot pricing, the jobs are estimated and every assembly unit is priced up and marked up then totaled up for a project price, one botton line contract price.
Have you ever seen a square foot of nm cable, or a square foot switch box? It sounds like the builder is trying to telling you what to do

Any resi job that specifically lists a bunch of stuff like dimmers, occpuancy sensors and the like are not spec jobs. They are customs, and customs just cannot be bid by the ft?.

so your saying individual circuits are additional cost?

The stuff I highlighted in red are not required by the NEC: (Smokes are not highlighted as they are most likely required by local building/fire codes, same with bath fans)
