I have never bid by the square foot, but am curious on how to do it. I have heard between $12 and $18 per square foot, but does that include material and profit and overhead?

The only realistic way to do it is to look over past similar work, and divide the price by the area.

Most living and working spaces are three dimensional. Pricing by volume might be more better.

Impossible to tell. In some places, $12/ft? will make you filthy rich, while in others you'll file for bankruptcy working for $18/ft?.
