Can I come out of the 480 /208 -120 transformer in to 2 fused disconnects (100 Amp each) or I have to terminate in one 200 Amp fused disconnect and than feed each 100 Amp separately?

You can run separate sets of conductors to separate fused disconnects from the transformer secondary. See 240.21(C).

I agree with david but might add you need to be sure that you maintain your transformer protection by properly sized overcurrent devices, primary or secondary, per 450.3 (see Table 450.3(B) + the notes)
