Study Question - Grounding Metal Handlhole Lids

Wondering on something. 314.30D requires bonding of metal handhole lids, for obvious reasons. What happens though, when someone is running cables and not individual conductors. No one would ever strip the casing off the cables to bond. Most sensible thing might be to run a separate ground wire in the conduit and bond to it. However, wouldn't 300.3B might seem to indicate this isn't allowed. Also, 300.3B might indicate a requirement to have a conductor for bonding the lid in each conduit, if there's multiple conduits coming from the source panel.
 
The requirement in 314.30 D points to 250.92 or 250.96A, 250.92 is for services , 4 methods and 250.96A is for feeders/BCs, a metal racway or wire type EGC

Well if someone is running cables and not individual conductors and not splicing in the pull boxes there would be no reason to strip the cable to bond to the lid. But 300.3B might seem to indicate running a separate individual ground wire for bonding the lid isn't allowed. Also, 300.3B might indicate a requirement to have a conductor for bon to indicate this isn't allowed. Also, 300.3B might indicate a requirement to have a conductor for bonding the lid in each conduit, if there's multiple conduits coming from the source panel.

Is it against the code if you’re running cabling to just run a separate individual ground wire and one conduit to bond to the lid?
