Subpanel without separate neutral and grounds

I own small duplex that 70 years old. The meter pan is on one side of the wall and the brealer box is on the other side.

I would like to upgrade with a combo meter/load center on the outside for added circuits without having to completely rewire the existing breaker box. The wiring is old and I don't want to touch it if possible.

In this case, do I really need separate neutral and grounds when the panel is a foot away?

Yes, the service disconnect will be at the meter so you need 4 wires downstream from that point, no matter what the length may be.
