There are way too many variable to give a specific answer.
Copper or aluminum wire? PVC conduit? Indoor disconnect or outdoors? Does your poco require rigid or shcedule 80 PVC on the pole? Does the poco provide the service lateral or do you? Does the poco provide the meter socket or do you? etc etc.....

I suggest making a detailed drawing and labeling the parts. Account for wall-mounting hardware, misc. parts like connectors and locknuts, no-ox if required/desired, etc.

Did you have the utility company engineers look at the job? they usually need the load data and site information before they come out, Did you call the line mark service to do a mark out? for the undergroung work, If you need someone to help you with a supply list,and price, you most likely should not be doing the work.
