Does medium voltage switchgear require to have the surge arrester for the electrical service?

I do not usually trust the utility to protect my equipment, be it MV or LV.

Why don't you want to supply your own arrestors?

Not required by any code that I'm aware of, but certainly good practice. The closer the surge arrestor is to the equipment, the better the protection.
