How do you know what switch gear terminals are a,b, and c phases or since it may not matter is there a standard like top to bottom = a,b,c phases?

Larry got it right above or if unclear you could probably too follow the bussing attachments of the load side of the meter to the bussing.
 
Where it really matters, is high leg systems. The high leg must be the center phase, so if in a gear that can be accessed from the back, the high leg is always the center buss bar.
