Does table 300.5, minimum cover requirements, include interior work such as chiseling a chase for a floor outlet. Would you still need 2" of concrete over pvc in that case. If not how much?

No, that table is for underground work not a raceway embedded in concrete.

300.5 Underground Installations.
(A) Minimum Cover Requirements. Direct-buried cable
or conduit or other raceways shall be installed to meet the
minimum cover requirements of Table 300.5.
