Tamper resistant receptacles 2011 CODE article ? ( for dwelling )

The way I read article 406-12 and 210-52 all 15 and 20 amp receptacles must be tamper resistant in a dwelling. 2011 code. is this right ?
Don't think there was major changes (I didn't look at 2011) but look at the exceptions.

All receptacles covered in 210.52 must be TR unless an exception is met. So what is typical that may not require TR?

Anything over 5.5 feet high.

Receptacle within dedicated space like behind an appliance that isn't easily moved.

Receptacles in crawl spaces aren't mentioned in 210.52.

Many just use TR on everything as that lessens the chance of getting the wrong one installed in a place that maybe only has two or three that don't have to be TR type.
