Before I can responsd someone closed the thread or removed it. Below is what I have in educational facility. I have 208V single phase circuit going to the special receptacle shown below from 208/120V three phase panelboard. I don’t have nema rating or blade picture of receptacles.

Would this fall under 15A or 20A 250V non locking type receptacle mentioned in 406.12 of NEC 2017 first paragraph?

It's impossible to say without more information. Why don't you just put a mark up on the plans to have the inspector check compliance with 406.12 on inspection? Why are you so scared to leave markups on plans? My plans typically have tons of markups and the inspector checks them in the field. Instead of fretting about all these things and wasting time, why don't you just put a mark up on the plans?

Would not on 208V system single phase only 250V receptacles apply? Their cant be any other receptacle? I dont understand
 
Did you find the NEMA configuration for a 208v 20 amp straight blade receptacle ?
 
You noticed there is no 208v, they use 250v.
You post states "educational" which is not definitive enough but since the 125v are designated as TR one would figure the 250v (208) would be also but, electrofelon notes, since the designer did not specify straight or locking you could do a RFI.
 
The UL listing of the receptacle is "up to 250V" so 208V application would fall into "up to 250V". What you pictured doesn't give enough info to tell which of the receptacles possible is to be used as far as blade configuration. So what Augie said, make a RFI, or what fellon said, do mark up.
